Advance Care Plan (CPT 99497)

• Covers healthcare proxy, durable power of attorney, living will, and medical orders for life-sustaining treatment.
• Can be combined with an Annual Wellness Visit to avoid extra co-pays.
• More than one provider can bill for this service for the same patient at the same time.
• No need for the patient’s written consent to bill for this, but providers should inform patients about any costs.
• Includes 30 minutes of face-to-face time with the patient, family, or surrogate.

Complex Chronic Care (CPT 99487)
